Terms & Conditions

 

Checsupport – NDIS Service Provider


Effective Date: 13/04/2025


Welcome to Checsupport. These Terms and Conditions (“Terms”) govern your access to and use of our services, website, and any interactions you have with us, including the delivery of NDIS supports. By engaging with Checsupport, you agree to these Terms.


 1. NDIS Participant Eligibility 

 

Checsupport provides services to individuals who are:


  • Approved NDIS participants, or 
  • Awaiting approval and assessment by the NDIS
     

All participants must have a current NDIS plan and funding allocated for the requested supports, or agree to private billing arrangements.


 2. Service Agreement 


 Before services commence, Checsupport and the participant (or their nominee/guardian) will enter into a written Service Agreement that outlines: 


 

  • The type of services to be delivered
     
  • The frequency and duration of supports
     
  • Pricing and payment terms
     
  • Responsibilities of both parties
     
  • Cancellation and notice terms


 3. Fees and Payments 


 

  • Checsupport charges in accordance with the NDIS Price Guide.
     
  • Invoices will be issued weekly, fortnightly, or as agreed.
     
  • Payments are expected within 7 days unless otherwise agreed.
     
  • Services may be paused if accounts are overdue.


 4. Cancellations and No-Shows 


Participants must provide at least 24 hours’ notice to cancel a scheduled service.


  • Less than 24 hours’ notice may result in a cancellation fee, in line with NDIS guidelines.


  • Repeated no-shows may lead to a service review.


 5. Rights and Responsibilities 


 Checsupport Agrees To: 

 

  • Provide quality services that are safe, respectful, and person-centred
     
  • Protect the privacy and confidentiality of all clients
     
  • Comply with all NDIS requirements and codes of conduct


 Participants Agree To: 

 

  • Treat staff with respect and dignity
     
  • Attend scheduled appointments or notify us of changes
     
  • Provide accurate and up-to-date information


 6. Privacy and Confidentiality 


Checsupport is committed to protecting your privacy. All personal information is handled in accordance with the Privacy Act 1988 (Cth) and relevant health records legislation. We will only share your information with your consent or when required by law. 


 7. Feedback and Complaints 


We value feedback and are committed to continuous improvement.

If you are not satisfied, you can also contact the NDIS Quality and Safeguards Commission. 


 8. Limitation of Liability 


Checsupport is not liable for indirect, incidental, or consequential damages that may arise from the use of our services, except as required under Australian law.


 9. Changes to These Terms 


We reserve the right to update these Terms and Conditions at any time. Updates will be posted on our website, and we encourage you to review them regularly.
